On December 20, Marta Pisano Gonzalez and Jose Ramon Hevia Fernandez presented the Active Patient Programme and the project EFFICHRONIC in the city of Aviles, Asturias.

During this talk, they highlighted that socioeconomic and cultural differences produce short- and long-term inequalities in relation to self-care and health. These inequities determine high-risk behaviors that have an effect on the use and adherence to health resources, and hence, on the impact of chronic diseases.
